Advanced Care Planning Presentation

8 out of 10 Canadians have never heard of Advance Care Planning. ACP is a process of reflecting on and communicating your wishes for end of life care with your family, friends and health team. Most families don’t know their loved one’s wishes — and may have some very difficult decisions to make if you aren’t able to speak for yourself due to illness or injury. Having discussions and recording your wishes is a priceless gift you can give your loved ones.  A lawyer is not required to fill out the forms which express your personal wishes. Learn more at this free presentation on Monday Oct. 17 from 7-9 pm at the church and don’t forget to register to guarantee a seat and sufficient resources.
